Activating Earthquake Coverage
1. The Signed Enrollment Form with no alternations to the property address, amount
of coverage, or premium. Altered forms are invalid. Changes or corrections must be
made and resubmitted through the website. You may increase coverage when
resubmitting, but you cannot decrease coverage offered; and
2. A Payment Made or Endorsed To The Surplus Lines Wholesaler, less any
applicable retain agent or broker fee. Insureds may make payment to the Surplus
Lines Wholesaler by check or money order, or you may submit an agency check for
the payment in full. Retail agent or broker fees submitted to the company will be
refunded to the insured.
3. Follow the Instructions on the Quotation & Enrollment form, and secure the
signature of applicant confirming there is no pre-existing earthquake damage to the
property they wish to insure.
DO NOT FAX APPLICATION AND/OR COPY OF CHECK TO BIND COVERAGE.
GEOVERA SPECIALTY DOES NOT BIND VIA FAX.

The Policy will become effective at 12:01 a.m. the day following receipt of payment by
GeoVera Specialty Insurance Services, Inc. unless a later Requested Effective Date is
entered on the enrollment form.
